If you have used the medication before but not with us, please select the required dose & attach Proof of Previous Usage (POPU) to your Health Form or send via email - this can be a copy of your prescription, a copy of your confirmation / order email from your provider or a photo of your current medication box which displaying your name, dose & date of order/prescription.
