Why do I need to complete a health form?

 

We need to ensure you are safe & suitable for treatment. Your information will never be shared with any 3rd parties beyond our pharmacy partners. Information is only ever shared with relevant professionals for your treatment.

PLEASE NOTE: a form of ID to verify you are over the age of 18 and that we are prescribing for a genuine person is required on the health form.

 

If you have used the medication before & are ordering a higher dose than the first dose, we will require proof of previous usage. If this information is not loaded with your health form, it may cause delays to your order.

What happens after I have done the form?

When you submit your form, you can either proceed to checkout to complete your purchase or you can wait for our team to verify your form & send you an invoice via email. If you choose to pay straight away and are deemed unsuitable for treatment, a full refund will be issued.
